Unlock Your Potential with Support Coordination
Our Support Coordination services are tailored to empower you within the NDIS framework. From demystifying your support budget to guiding you through the intricacies of your NDIS plan, our dedicated support coordinators are with you every step of the way. We help you map out and manage your supports, identifying the right individuals, whether family members or external services, to assist in achieving your goals.
As your needs evolve, we adapt and refine your support network. Moreover, as support coordinator I can equip you with the skills to independently manage your supports, granting you greater control over your journey towards achieving your goals and fostering independence. And when life throws unexpected challenges your way, we're here to help you and your support circle navigate them, whether it's a hospitalisation or a relocation.
Specialist Support Coordination
Specialist support coordination is a higher level of support. The focus is on reducing complexity in the participant's support environment and helping the participant overcome immediate and/or significant barriers in plan implementation.
Barriers include a interface with other government agencies such as Housing, Health and Legal. This is our area of expertise.
Social Work
Social workers play a vital role in addressing a wide range of social issues, including addiction, gambling, poverty, homelessness, mental health, domestic violence, physical disabilities, social injustice, trauma, legal matters, relationships, and self-determination.
Social workers also advocate on behalf of individuals, assisting with tasks such as preparing submissions, attending court proceedings, writing support letters, and making referrals to health and community services. Their comprehensive support helps individuals navigate complex challenges and work towards personal fulfilment and well-being.
